As endoscopic appproaches to the anterior skull base, using extended transnasal techniques have become more popular and widespread, there is controversy over the optimal utilization of th is method. This includes the selection of patients, operative and postoperative management, incidence and nature of complications, and effect and prognosis related to the disease process treated. Advance have also occurred in the more tradtional craniotomy methods, and both approaches must be balanced individually in the treatment of each individual patient. This commentary attempts to provide a balanced perspective.
